Eligibility

Students who are taking at least 9 semester hours that include at least one daytime hour and who pay the health fee are entitled to the services of the Student Health Center.

To receive clinic services, it is required that a completed medical/health history form (including immunization record) be on file.  To avoid a delay at doctor's hours, stop in or call to check if your records are complete.  Medical/health history forms are available through the Student Health Department or click here to download the medical/health history form. Please print this form and have your doctor sign it before submitting it.

Student Health Insurance

All full-time students at Morningside College are required to submit proof of accident and health insurance coverage before validating for classes.  If currently on an individual or family plan, the student should be knowledgeable about the use of their insurance plan before coming to college.  If the student is not covered under another family or individual plan, an insurance plan designed especially for Morningside College students is offered.  Information on that plan can be found at:  www.sas-mn.com/.   Claim forms may also be found there.  On campus, questions in regard to the student health insurance plan may be directed to the office of Student Services.

Confirmation of Illness

Although your Health Center cannot excuse you from work or classes, it can issue a statement that verifies that you visited the Health Center because of illness or injury.  To maintain confidentiality, the nature of your problem is not disclosed.  In the event that medical problems conflict with your academic schedule, it is your responsibility to speak with your faculty and dean.  No written statement will be issued unless a visit is made to the Health Center.
