Faculty Exam Accommodation Form

The Americans with Disabilities Act of 1990 and Section 504 of the Rehabilitation Act of 1973 require educational institutions to provide reasonable academic adjustments to students with documented disabilities. Verification of the student's disability and need for academic adjustments is on file in Student Services.

By law, alternate exam arrangements may be made for students who have documented disabilities.

All exam accommodations for students with documented disabilities will be provided by Disability Services unless the faculty chooses to provide the necessary accommodations. Exams may begin as early as 8:00 a.m. and must be completed by 4:30 p.m. Monday - Friday. Any exams scheduled outside these hours must be accommodated by the faculty.

INSTRUCTIONS:
  • It is the student's responsibility to complete the Exam Accommodation Request form at least three days prior to the exam. The online form will generate an automated request which is emailed to the professor.
  • The professor must then complete the Exam Accommodations form provided below.
