Click on the link below to download the Textbook adoption form. Please follow these 3 steps:

1. Open the file and fill it out using Microsoft Word. Your tab key works great to work your way through the form.
2. When you are finished, use the "Save as" function. Name the file with three words: "textbook", your course number, and the term. For example, "Textbook POLS320 Fall02". This helps us to organize your request.
3. Send the file as an email attachment to Denise Simons.
